The Miya Model can accommodate a wide variety of gynecological procedures. In this article, we provide tips and tricks for performing certain OBGYN procedures using the Miya Model.

Endometrial Biopsy

We recommend filling the inside cavity with gel or jello and placing rubber bands around the cervix in order to create resistance.

Dilation and Curettage

To narrow the opening to the uterus, place a rubber band around the cervical os. 

Measuring Cervical Length

Create true-to-life resistance by placing rubber bands around the cervix.

Colposcopy

You can use any marking pen to practice pathology on the cervix.

Hysteroscopy

Cut meat into small 2-3 centimeter chunks. Then, use a Keith needle (or any long sewing needle) and thread to create a “popcorn string” of meat. Insert the threaded meat into the cavity so it is flush against the cavity wall.

This practice is ideal for an energy operative hysteroscopy. And you can also repeat this technique for MyoSure resection practice with used uterus chunks.

Generic Sling Training (with any sling)

Before beginning the procedure…

  • Apply Surgilube to the trocar to help reduce friction.
  • Practice disassembling and reassembling the model to become familiar.
  • Review bony landmarks, identifying the pubic tubercles, pubic rami, and ischial spines.
  • Review the soft tissue components, locating the mid urethra, white line-arcus tendineus fascia pelvis (ATFP), obturator muscles, and sacrospinous ligament.

Then, follow these tips for effective sling practice:

  1. Begin practicing with bony pelvis only, removing the vulva, vagina, uterus, and bladder.
  2. Do several passes to understand the trocar trajectory.
  3. Once comfortable using only the bony pelvis, reinsert the vagina. Do several passes in to maximize familiarity with the full-thickness vaginal wall dissection.
  4. Finally, add the vulva to practice the full procedure.

Altis® Single-Incision Sling

First, you must review the critical differences of the Altis sling:

  • Trocar shape – The Altis sling has a tighter helical curve than other Transobturator Tape (TOT) slings and will “Hug” the pubic rami bone closer.
  • Dissection tract – full thickness vaginal wall dissection will help minimize mesh erosion risks
  • Anchor placement – The Altis sling goes through the OM, and you should feel the “Pop”
  • Adjustability – The Altis can be tightened or loosened prior to cutting the loop off.
